Your initial appointment is going to consist of a comprehensive examination and assessment of your treatment options. Your first appointment is going to include exclusive time with Dr. Thurm, so that he can diagnose you and provide you with treatment options to help you get that great smile.
Please assist us by giving the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the required completed forms. This can save time and enable us to help you process any claims on your behalf.
Please note: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office in case you have a medical condition which could be of concern before treatment (i.e., diabetic issues, higher blood stress, synthetic heart and soul regulators & joints, rheumatic fever, etc.) or in case you’re currently taking any prescribed medication (i.e., anticoagulant therapy, aspirin, heart medications, etc.).
If your physician or dentist has taken x rays, you might request that they forward them to our office. If there’s not sufficient time, make sure you pick them up and bring them to our office. If additional x rays are needed, they can be taken at our facility.
